Insurance Plan Participation
INSURANCE COVERAGE
Your insurance coverage is a contract between you and your insurance company.
It is your responsibility to know your coverage details, including but not limited to your deductible and coverage limits.
However, as a courtesy to our patient, we will call your insurance carrier to determine your level of coverage, verify your deductible, co-pay and exclusions.
For this reason, please bring all your applicable insurance cards with you to your first visit.
BILLING AND FEES
For your convenience, we take care of billing your insurance company for you.
Our fees are established within the usual and customary limits for physical therapy treatment in our area.
Our policies regarding fees are clearly defined and discussed with you upon your first visit.
Please fill read and sign our patient financial information form prior to your first visit
CASH RATES OR NO INSURANCE
M and M accepts most insurance plans.
A cash rate is available in the event of no insurance coverage or for massage therapies.

NOTES:
- Most plans also have an out-of-network benefit plan, other than HMO’s, with a slightly higher co-pay or deductible rate.
- We are not contracted with any HMO plans, so if you have an HMO, you will have to stay within your network for coverage.
- We offer payment plans and up front cash rates if patients are interested in paying out-of-pocket at a lower rate.
